Referring a patientAnyone can make a referral for all or any part of our services, with the patients' permission. The Care team will process the referral, including contact with the patients' own GP. The minimum age for referrals is 18 years. Beaumond House operates over an approximate radius of 15 miles from Newark. The area it therefore encompasses include: Tuxford, Bilsthorpe, Ollerton, Sutton-on-Trent, Thorpe-on-the-Hill, Leadenham, Caythorpe, Long Bennington, Bottesford, Bingham, Lowdham, Southwell, Collingham and Bassingham. Completed forms can be faxed to our safe haven fax number 01636 613262 or scanned and emailed to referral@beaumondhouse.co.uk Each patient remains under the care of their own GP whilst accessing any service at Beaumond House. All information received about patients or carers is kept confidential. Following the initial referral, arrangements will be made for the patient to have a full assessment in order to ensure the appropriate services are offered as soon as reasonably possible. |
