Service Lead for the Integrated Hub and Hospice at Home
Full time
£32.15 an hour
The Integrated Hub in John Eastwood Hospice, Mansfield.
Role Purpose
You provide senior operational and clinical leadership for the End-of-Life Care Together Hub. You hold delegated responsibility for service delivery, workforce management, quality, performance, and use of resources on behalf of Beaumond House Hospice and the Alliance.
You lead integrated teams to support patient choice, enable people to remain in or return to their preferred place of care, reduce avoidable admissions, and improve discharge flow for people who are rapidly deteriorating.
You work collaboratively with system partners to align services to agreed pathways, strengthen integrated working through neighbourhood teams, and deliver continuous improvement in the quality and safety of end-of-lifecare.

Clinical Educator and Quality Lead
37.5 hours per week - Monday to Friday, with the flexibility to work occasional weekends where required by the service.
£23.50 an hour
Primary base is John Eastwood Hospice in addition to various bases across North Nottinghamshire
Role Purpose
The purpose of this role is to strengthen the clinical skills, quality and safety of Hospice at Home and the End-of-Life Care Together Integrated Hub. Nurse educators combine clinical expertise and a passion for teaching.
This role provides visible clinical leadership, education and quality oversight.
You will:

Design, implement, evaluate and revise clinical education for registered and unregistered staff. You will support staff to deliver evidence based, person centred care in a changing health care environment.

Lead on key quality functions. This includes clinical audit, incident and complaint learning, service improvement, reporting, and support for research activity in Hospice at Home and the Hub.

Play a key part in CQC readiness and ongoing compliance. You will help ensure that education, practice and evidence align with regulatory and local quality standards.

Deputise for the Service Lead when required.

Support coordination of the Hub and Hospice at Home within an agreed level of responsibility.
