Health Care Assistant - Hospice at Home, Mansfield

Role Purpose

We are expanding our Hospice at Home service and are seeking compassionate, committed people to join our growing team. This role offers the opportunity to support patients and families in their own homes as the service extends across North Nottinghamshire.

The Hospice at Home service provides care for palliative patients seven days a week, from 7am to 9pm. Shifts include a mix of shorter shifts and 12-hour shifts.

To provide individual high-quality care for patients in their own homes, including appropriate support for families, in line with hospice policy and agreed care plans. Care may include supporting patients while symptoms are assessed and brought under control, caring for the dying, supportive psychosocial and spiritual care in line with any agreed plan, enabling patients’ independence and supporting discharge home.

Working under the direction of a registered nurse and as part of the Beaumond House team and the End-of-Life Care Together Integrated Hub you will help maintain the standard of excellence, so the philosophy and values of Beaumond House Hospice Care are met.

 

We offer a range of established part time contracts of 25.5, 26.25, 27, 27.75, 28.75, 30.12, 30.54, 30.62, and 34.5 hours per week. Applicants should state their preferred contracted hours as part of the application process.

Service Lead for the Integrated Hub and Hospice at Home

Full time

£32.15 an hour

The Integrated Hub in John Eastwood Hospice, Mansfield.

Role Purpose

You provide senior operational and clinical leadership for the End-of-Life Care Together Hub. You hold delegated responsibility for service delivery, workforce management, quality, performance, and use of resources on behalf of Beaumond House Hospice and the Alliance.

You lead integrated teams to support patient choice, enable people to remain in or return to their preferred place of care, reduce avoidable admissions, and improve discharge flow for people who are rapidly deteriorating.

 

You work collaboratively with system partners to align services to agreed pathways, strengthen integrated working through neighbourhood teams, and deliver continuous improvement in the quality and safety of end-of-lifecare.

Care Navigator


Full time

The Integrated Hub in John Eastwood Hospice, Mansfield

Role Purpose

To work within the End-of-Life Care Together Hub to provide coordinated, compassionate support to patients in the last year of life, including those in their final days.

 

The role focuses on ensuring patients, carers and professionals can access the right services at the right time, supporting admission avoidance and timely discharge planning in line with individual Advance Care Plans.

 

Through effective care coordination, communication, administration and data management, the Care Navigator supports the work of clinical teams across the End-of-Life Care Together Alliance, while upholding the values, standards and ethos of Beaumond House Hospice Care.

Payroll Officer

Beaumond House Hospice Care, Newark

Role Purpose

This is a key role ongoing and during a period of organisational change, requiring someone with prior payroll experience, confidence, and the ability to develop and shape the role to meet evolving needs.

 

You will work closely with HR, Finance, and Clinical teams to resolve issues quickly, maintain excellent communication and help streamline processes across the organisation.

 

You will be required to understand and support the staff rota system helping to ensure continuity of support with our daily operations.

Role Purpose

To work collaboratively with the Beaumond House team and the End-of-Life Care Together Integrated Hub to deliver a consistently high standard of hospice care in line with the organisation’s philosophy and values.

The role focuses on planning, coordinating, delivering and evaluating person-centred, evidence-based palliative care for patients, families and carers.

The Registered Nurse will support the effective delivery of the Beaumond House Hospice at Home service within their caseload, working closely with the multi-disciplinary team and partner agencies, contributing to staff and service development, and supporting admission avoidance and timely discharge to enable people to remain at home in accordance with their preferences and care plans.

Working at Beaumond House Hospice Care means working in a passionate, supportive team environment with care at the heart of all we do.

We care for people within the Newark and Sherwood offering hospice at home, day services and in-patient care as well as other support services.

We are a partner organisation within the End of Life Together Mid Notts Alliance. Together with Nottinghamshire Hospice, Nottinghamshire Healthcare, Sherwood Trust Hospitals, Primary Integrated Services Ltd (PICS) and Cruise, we support patients who are in their last year of life.
